**ROC-101** is a first-in-class, investigational oral **pan-Rho-associated protein kinase (ROCK) inhibitor**. It potently inhibits both ROCK1 and ROCK2 isoforms, producing anti-proliferative, anti-inflammatory, and anti-fibrotic effects. The drug is being developed primarily for **pulmonary arterial hypertension (PAH)** and **pulmonary hypertension associated with interstitial lung disease (ILD-PH)**, both life-threatening cardiopulmonary conditions with limited disease-modifying treatments. The mechanism targets pathological pulmonary arterial remodeling and pulmonary fibrosis, aiming to promote vasodilation, improve endothelial function, and reduce vascular remodeling, complementing existing PAH therapies. ROC-101 is administered orally and has completed favorable Phase 1 safety and tolerability studies; Phase 2a trials in PAH and ILD-PH are commencing in late 2025[1][3][5][7][8].
