CMN-001 (rocapuldencel-T; formerly AGS-003) is a personalized dendritic cell-based immunotherapy developed for the treatment of metastatic renal cell carcinoma (mRCC). The therapy uses autologous matured dendritic cells that are electroporated with in vitro transcribed RNA derived from the patient’s own tumor. This process enables the dendritic cells to present a broad array of tumor antigens unique to each patient. The mechanism of action involves induction of an adaptive T cell response specifically directed against these tumor antigens. CMN-001 delivers three critical signals for T-cell activation: presentation of tumor antigens via HLA molecules to activate cytotoxic T lymphocytes (CTLs), costimulatory signaling through CD80/CD86 engaging CD28 on T cells, and additional stimulation via CD40L RNA leading to IL-12 secretion and expansion/function of antigen-specific memory CTLs. Clinical development has included phase 3 and ongoing phase 2b studies in mRCC, with evidence suggesting synergy when combined with everolimus or checkpoint inhibitors[2][4][6].
