Overview

Romiplostim is a fusion protein analog of thrombopoietin, classified as a thrombopoietin receptor agonist. It is used to increase platelet counts in adults and children (aged 1 year and older) with immune thrombocytopenia (ITP) who have not responded adequately to other treatments, such as corticosteroids or splenectomy. Romiplostim works by binding to and activating the thrombopoietin receptor (c-Mpl) on megakaryocyte precursors in the bone marrow, stimulating intracellular signaling pathways—including JAK2/STAT5, ERK1/2, and AKT—that promote cell proliferation, maturation of megakaryocytes, and increased platelet production. Unlike endogenous TPO, romiplostim has no sequence homology with natural TPO but mimics its function through its peptibody structure. It is also approved for use in patients exposed to high levels of radiation (acute radiation syndrome). Romiplostim was developed by Amgen and received FDA approval in August 2008.
