Ropeginterferon alfa‑2b is a long‑acting, mono‑pegylated type I interferon used primarily for the treatment of polycythemia vera, a slow-growing blood cancer characterized by excessive red blood cell production. It acts by binding to the interferon alpha/beta receptor (IFNAR), activating downstream JAK/STAT signaling pathways that result in anti-proliferative, antiangiogenic, pro-apoptotic, and immunomodulatory effects. These include augmentation of T-cell, macrophage, and natural killer cell activity. The drug is a recombinant form of interferon alpha‑2b conjugated via proline to polyethylene glycol (PEG), which prolongs its half-life and allows less frequent dosing compared to conventional interferons. Ropeginterferon alfa‑2b has demonstrated efficacy in reducing JAK2-mutant allele burden in patients with polycythemia vera and has also shown clinical activity in essential thrombocythemia and moderate COVID‑19[1][5][6][8].
