ropivacaine + lidocaine + adrenaline is a multi-component local anesthetic solution used for regional anesthesia, specifically in brachial plexus blocks for patients undergoing surgery for upper extremity fractures. This combination leverages the distinct pharmacological profiles of two aminoamide local anesthetics: lidocaine, which provides a rapid onset of action, and ropivacaine, which offers a longer duration of sensory block with a more favorable safety profile regarding cardiac and central nervous system toxicity. Both agents function by inhibiting voltage-gated sodium channels on the neuronal membrane, preventing the propagation of action potentials and thereby blocking nerve conduction. The addition of adrenaline (epinephrine) serves as a sympathomimetic vasoconstrictor, acting primarily on alpha-adrenergic receptors to reduce local blood flow. This slows the systemic absorption of the anesthetics, extending the duration of the block and minimizing the risk of systemic toxicity. This specific combination was utilized in clinical research at the Nepal Medical College and Teaching Hospital to evaluate the efficacy of regional anesthesia and to serve as a baseline for comparing the effects of adjuvants like dexmedetomidine.
