ropivacaine + lidocaine + dexamethasone + epinephrine is a multi-component analgesic solution developed by the Medical University of Gdansk for use in ultrasound-guided ilioinguinal and iliohypogastric (ILIH) nerve blocks. This combination therapy is specifically designed to manage procedural pain in patients undergoing transfemoral transcatheter aortic valve implantation (TF-TAVI). The formulation leverages the synergistic effects of two local anesthetics—ropivacaine (long-acting) and lidocaine (intermediate-acting)—to provide rapid and sustained blockade of sodium channels in peripheral nerves. Dexamethasone is included as a corticosteroid adjuvant to prolong the duration of sensory blockade, while epinephrine (adrenaline) acts as a vasoconstrictor to limit systemic absorption, reduce toxicity, and further extend the local anesthetic effect. The Medical University of Gdansk has evaluated this mixture in prospective randomized trials to compare its efficacy against standard local anesthesia infiltration.
