Overview

ropivacaine + tranexamic acid is a pharmacological combination consisting of a long-acting amide-type local anesthetic and a synthetic derivative of the amino acid lysine that acts as an antifibrinolytic. This combination is frequently employed as a periarticular cocktail injection or topical application during orthopedic procedures, specifically total knee arthroplasty (TKA). Ropivacaine provides regional analgesia by reversibly blocking sodium channels in nerve fibers, thereby inhibiting the conduction of pain impulses. Tranexamic acid reduces surgical and postoperative blood loss by competitively inhibiting the activation of plasminogen to plasmin, which prevents fibrinolysis and stabilizes clot formation. Clinical research, including studies at The First Affiliated Hospital of Bengbu Medical College, has evaluated this combination for its ability to enhance postoperative recovery by simultaneously addressing pain management and hemostasis.
