Rose bengal is a synthetic xanthene dye with the chemical formula C20H2Cl4I4Na2O5 (commonly as the disodium salt)[6][5]. It is widely used as a biological stain and diagnostic agent in ophthalmology to detect damaged or devitalized cells on the ocular surface[2][6]. In research and clinical settings it acts as a photosensitizer—upon light activation it generates singlet oxygen and other reactive oxygen species that can induce cell death or microbial killing[1][4]. Rose bengal has been studied for photodynamic therapy applications in oncology and infectious diseases due to its ability to produce cytotoxic effects under illumination[1][2]. Mechanistically it inhibits vesicular glutamate transporter (VGlut) and synaptic vesicle monoamine transporter (VMAT), with reported Ki values of 19 nM and 64 nM respectively[1]. It also inhibits cytochrome P450 isoforms CYP3A4/5 and UDP-glucuronosyltransferase UGT1A6 in a light-dependent manner[1].
