Rose bengal I-131 is a radiopharmaceutical diagnostic agent composed of the dye rose bengal labeled with radioactive iodine (Iodine 131). It has been primarily used as a diagnostic aid for hepatic function determination, particularly in liver imaging and the differential diagnosis of jaundice. After intravenous administration, it is selectively taken up by hepatocytes and excreted into bile, allowing visualization of liver parenchyma and biliary tract via gamma camera imaging. This enables assessment of hepatic function, detection of lesions such as abscesses or tumors, and evaluation of biliary obstruction. The compound is classified under ATC code S01JA02 (diagnostic agents—coloring agents) and has been approved for clinical use in this indication[2][4][5]. The mechanism involves its uptake by functioning hepatocytes followed by excretion into bile; thus, areas with impaired uptake or excretion can be visualized on scans[5]. Historically manufactured using commercial-grade processes, pharmaceutical-grade production methods have since been developed to meet regulatory standards[6].
