Overview

Roxithromycin is a semi-synthetic 14-membered ring macrolide antibiotic derived from erythromycin. It exhibits a similar antibacterial spectrum to erythromycin but offers improved pharmacokinetic properties, including enhanced acid stability and superior tissue penetration. Its primary mechanism of action involves binding to the 50S subunit of the bacterial ribosome, thereby inhibiting protein synthesis. Beyond its established use in treating respiratory tract, urinary tract, and skin infections caused by Gram-positive and certain Gram-negative organisms (such as *Mycoplasma pneumoniae* and *Chlamydia*), roxithromycin has demonstrated significant anti-inflammatory, antiangiogenic, and potential antitumor activities. These non-antibiotic effects are thought to be mediated through the inhibition of oxidative stress, nitric oxide production, and the activation of nuclear factor kappa B (NF-κB). Clinical research has also explored its efficacy in chronic inflammatory conditions such as rheumatoid arthritis and chronic obstructive pulmonary disease (COPD).
