RVG-conjugated MSC-Exo is an engineered exosome-based therapeutic agent consisting of **mesenchymal stem cell-derived exosomes** (MSC-Exo) chemically conjugated with the **rabies viral glycoprotein (RVG) peptide** via a DOPE-NHS linker. The RVG peptide imparts central nervous system-targeting properties, significantly **enhancing the delivery of exosomes to brain tissue** after systemic administration. Preclinical studies in mouse models of Alzheimer’s disease have shown that RVG-conjugated MSC-Exo crosses the blood-brain barrier, reduces cerebral amyloid-β plaque deposition, lowers pro-inflammatory cytokines, increases anti-inflammatory cytokines, and improves learning and memory performance. The **main mechanism of action** is believed to be the targeted delivery and modulation of neuroinflammatory responses by transferred exosomal RNA and proteins, providing both neuroprotective and immunomodulatory effects[1][3][5][7].
