RVG-EVs + Echi (also known as RVG-EVs@Echi) is an engineered nanotherapeutic system designed for brain-targeted delivery in the treatment of Parkinson's disease. It consists of extracellular vesicles (EVs) functionalized with a rabies virus glycoprotein (RVG)-derived peptide, which facilitates crossing the blood-brain barrier (BBB) and selective accumulation in dopaminergic neurons and microglia. The vesicles are loaded with echinatin (Echi), a small molecule NLRP3 inflammasome inhibitor isolated from Glycyrrhiza species. The therapeutic mechanism involves restoring dopaminergic neuron survival through IGF-2–mediated activation of the PI3K/Akt/Nrf2 pathway, reducing neuroinflammation and oxidative stress, and improving motor function in preclinical models.
