This is a combination therapy consisting of three distinct medications used together for the treatment of non-small cell lung cancer (NSCLC) patients with idiopathic pulmonary fibrosis (IPF). The combination leverages: 1. Carboplatin - An organoplatinum antineoplastic alkylating agent 2. S-1 - An oral fluoropyrimidine anticancer drug 3. Pirfenidone - An antifibrotic agent with anti-inflammatory and antioxidant properties This combination has been studied for its potential to safely treat NSCLC patients with IPF, as these patients are at high risk of acute exacerbation of IPF when undergoing chemotherapy. Clinical research has shown that pirfenidone may help prevent chemotherapy-associated acute IPF exacerbations when combined with carboplatin-based chemotherapy regimens[1][2]. In clinical studies, patients received pirfenidone continuously while being treated with carboplatin and S-1 as first-line chemotherapy. After disease progression, patients could receive other cytotoxic agents or immune checkpoint inhibitors while continuing pirfenidone[2]. The combination has demonstrated promising efficacy with manageable toxicity profiles. Notably, none of the patients in key studies developed acute exacerbation of IPF associated with first-line chemotherapy when using this combination[1][2].
