S 41015 is an orally bioavailable small molecule prodrug developed by Servier (Institut de Recherches Internationales Servier) for the treatment of cardiovascular diseases, specifically thromboembolic disorders. It is the ethyl ester derivative of the active metabolite S 38844, which acts as a potent and selective direct thrombin inhibitor (DTI). By inhibiting thrombin (Coagulation Factor IIa), the active form prevents the conversion of fibrinogen to fibrin, thereby blocking the final step of the coagulation cascade and preventing clot formation. S 41015 was evaluated in Phase 1 clinical trials using microdosing strategies to determine its absolute bioavailability and metabolic profile in humans, as well as its potential for drug-drug interactions with CYP3A4 inhibitors such as ketoconazole.
