Overview

S-Ethylisothiourea is a small molecule inhibitor of nitric oxide synthase (NOS), with particular selectivity for the inducible isoform (iNOS). It acts by competitively binding to the L‑arginine site of NOS enzymes, thereby blocking the production of nitric oxide and reducing vasodilation and other effects associated with excessive NO synthesis. S-Ethylisothiourea has been studied in models of sepsis, inflammation, and acute hypotension. Its clinical use includes treatment for acute arterial hypotension due to shock, trauma, poisoning, or surgical interventions—especially when adrenomimetics are contraindicated or ineffective. The drug increases peripheral vascular resistance and improves cardiac function without stimulating the sympathetic nervous system or causing tachycardia. It also exhibits anti-inflammatory and analgesic properties and has been used in some regions under the brand name Difetur for antihypotensive indications[2][3][5][6].
