S-mephenytoin is the S-enantiomer of mephenytoin, a hydantoin-derivative anticonvulsant drug used in the treatment of various forms of epilepsy, especially refractory partial epilepsy. It acts primarily by blocking voltage-dependent neuronal sodium channels, stabilizing neuronal membranes and inhibiting the spread of seizure activity in the motor cortex. Additionally, S-mephenytoin is widely used in biomedical research as a specific probe substrate for Cytochrome P450 2C19 (CYP2C19), facilitating the study of genetic polymorphisms in drug metabolism[1][5][10].
