S4613 is a small molecule ryanodine receptor 2 (RyR2) stabilizer, belonging to a class of compounds known as "Rycals." Developed by Servier, it is primarily investigated for the treatment of cardiovascular conditions characterized by intracellular calcium handling abnormalities, such as heart failure and catecholaminergic polymorphic ventricular tachycardia (CPVT). The drug's mechanism of action involves stabilizing the closed state of the RyR2 channel in the sarcoplasmic reticulum of cardiac myocytes. It achieves this by enhancing or restoring the binding of the regulatory protein calstabin2 (FKBP12.6) to the RyR2 complex. This stabilization prevents the pathological leakage of calcium ions during the diastolic phase of the cardiac cycle, which is a key driver of triggered arrhythmias and contractile dysfunction in failing hearts. By normalizing intracellular calcium handling, S4613 aims to improve cardiac efficiency and prevent life-threatening ventricular arrhythmias.
