Sabatolimab + canakinumab is a combination of two monoclonal antibodies: *sabatolimab* (MBG453), an immunotherapy targeting T cell immunoglobulin mucin-3 (TIM-3), and *canakinumab* (ACZ885), an anti-interleukin-1 beta (IL-1β) monoclonal antibody. Sabatolimab inhibits TIM-3, a negative immune regulator expressed on leukemia stem cells (LSCs), blasts, and immune cells, leading to antileukemic activity through direct targeting, blockade of self-renewal signals (TIM-3/galectin-9), and antibody-dependent cellular phagocytosis of cancer cells[5][2][3]. Canakinumab blocks IL-1β, a pro-inflammatory cytokine involved in immune dysregulation and cancer pathogenesis, thereby reducing inflammation and potentially modifying the tumor microenvironment. The combination is investigated for *lower-risk myelodysplastic syndrome (MDS)*, with the rationale to provide synergistic immune modulation for more effective treatment[3].
