Sabcomeline hydrochloride is a small-molecule, orally active, functionally selective muscarinic M1 receptor partial agonist that was developed primarily for symptomatic treatment of cognitive impairment in Alzheimer's disease and other cognition/depressive disorders. It binds with similar affinity to all five muscarinic acetylcholine receptor subtypes but shows functional selectivity for M1, aiming to enhance cortical and hippocampal cholinergic neurotransmission and improve memory and executive function while minimizing peripheral cholinergic side effects.[1][2][7][9] Originally discovered and advanced by SmithKline Beecham/GlaxoSmithKline, sabcomeline reached phase III trials in Alzheimer's disease under the brand name Memric but was discontinued after poor efficacy and tolerability, and was later repositioned by BrainCells and Proximagen for cognitive and depressive indications before global development was halted.[1][5][6][9][10]
