Overview
Saccharomyces boulardii is a nonpathogenic, orally administered probiotic yeast used in microbiota-directed management of diarrheal disorders. Commercial and clinical products are strain-specific; the best-characterized pharmaceutical strain is **Saccharomyces boulardii CNCM I-745**, originally developed and manufactured by Biocodex. It acts locally in the gastrointestinal tract through strain-dependent effects on epithelial barrier integrity, intestinal immune signaling, pathogen and toxin activity, and gut microbial ecology. Clinical evidence is most established for prevention or adjunctive management of antibiotic-associated diarrhea and other acute or infectious diarrheal conditions, while evidence for inflammatory bowel disease maintenance has been inconsistent; a 52-week randomized Crohn disease study did not show a significant relapse-prevention benefit.
