Saccharomyces boulardii CNCM I-457 is a probiotic yeast strain investigated for its efficacy in preventing antibiotic-associated diarrhea (AAD), particularly in pediatric populations. As a yeast-based probiotic, it is inherently resistant to antibacterial agents, making it suitable for co-administration with antibiotics to preserve the intestinal microbiota. The mechanism of action includes the inhibition of pathogen adhesion, neutralization of bacterial toxins (such as those produced by Clostridium difficile), and trophic effects on the intestinal mucosa that enhance barrier function and enzymatic activity. It also modulates the host immune response by increasing the production of secretory IgA. While this specific strain designation (CNCM I-457) appears in clinical trial registries, it is frequently associated with studies evaluating the well-characterized CNCM I-745 strain.
