Sacituzumab govitecan + sacituzumab tirumotecan + toripalimab is a combination regimen being investigated as a neoadjuvant therapy for patients with resectable locally advanced oral squamous cell carcinoma (OSCC). The regimen incorporates two trophoblast cell-surface antigen 2 (TROP-2)-directed antibody-drug conjugates (ADCs)—sacituzumab govitecan (Trodelvy) and sacituzumab tirumotecan (SKB264)—alongside the PD-1 immune checkpoint inhibitor toripalimab (Tuoyi). Sacituzumab govitecan utilizes an SN-38 payload, while sacituzumab tirumotecan employs a camptothecin derivative; both payloads function as DNA topoisomerase I inhibitors to induce tumor cell death following TROP-2-mediated internalization. Toripalimab enhances the immune system's ability to recognize and attack malignant cells by blocking the PD-1/PD-L1 pathway. This triplet or alternating ADC approach is currently being evaluated in a Phase II clinical trial sponsored by Shanghai East Hospital to improve pathological response rates prior to radical surgery.
