Sacituzumab tirumotecan + envafolimab is an investigational combination therapy consisting of a TROP2-directed antibody-drug conjugate (ADC) and a PD-L1-directed immune checkpoint inhibitor. Sacituzumab tirumotecan (also known as MK-2870 or SKB264) comprises a humanized anti-TROP2 monoclonal antibody conjugated to a belotecan-derived topoisomerase I inhibitor payload (KL610023) via a proprietary sulfonyl pyrimidine-thiol linker, with a drug-to-antibody ratio (DAR) of approximately 7.4. Envafolimab is a recombinant humanized single-domain antibody (nanobody) that targets PD-L1. This combination is being evaluated in Phase II clinical trials for the treatment of patients with TROP2-positive advanced biliary tract cancers who have progressed on prior immunochemotherapy, aiming to leverage the cytotoxic effects of the ADC alongside the immune-potentiating effects of PD-L1 blockade.
