Sacubitril is an antihypertensive drug and a prodrug that, after activation to sacubitrilat (LBQ657), acts as a neprilysin inhibitor. Neprilysin is an enzyme responsible for the degradation of natriuretic peptides such as atrial natriuretic peptide and brain natriuretic peptide, which help lower blood pressure by reducing blood volume. By inhibiting neprilysin, sacubitril increases levels of these peptides, resulting in vasodilation, natriuresis (sodium excretion), diuresis (increased urine production), and reduced cardiac remodeling. Sacubitril is primarily used in combination with valsartan for the treatment of heart failure with reduced ejection fraction but can also be found in other combinations or studied alone[5][1].
