DKF-458 is a generic formulation of sacubitril and valsartan (originally marketed as Entresto), developed by Dongkook Pharmaceutical. It is a combination of sacubitril, a neprilysin inhibitor, and valsartan, an angiotensin II receptor blocker (ARB). This dual mechanism of action, known as an angiotensin receptor-neprilysin inhibitor (ARNI), works by simultaneously inhibiting neprilysin to increase levels of beneficial natriuretic peptides and blocking the angiotensin II type 1 (AT1) receptor to prevent vasoconstriction and fluid retention. DKF-458 is developed for the treatment of chronic heart failure and essential hypertension, and has completed bioequivalence clinical trials in South Korea.
