Sacubitrilat is the active metabolite of the prodrug sacubitril. It acts as a potent neprilysin inhibitor. Neprilysin is a neutral endopeptidase responsible for degrading several vasoactive peptides, including atrial natriuretic peptide (ANP), brain natriuretic peptide (BNP), and c-type natriuretic peptide (CNP). By inhibiting neprilysin, sacubitrilat increases levels of these peptides, leading to vasodilation, natriuresis, diuresis, and inhibition of pathological growth and fibrosis in cardiovascular tissues. Sacubitrilat is not administered directly as a drug; it is formed in vivo after administration of sacubitril-containing medications such as the combination product with valsartan used for heart failure[5][7].
