Sage leaf (*Salvia officinalis*) is a botanical preparation being evaluated for its potential to improve cognitive function in patients with Parkinson's disease and mild cognitive impairment (PD-MCI). In a Phase 4 pilot controlled clinical trial sponsored by the Newcastle Upon Tyne Hospitals NHS Foundation Trust, sage leaf is compared against vitamin E to assess its efficacy in addressing cognitive deficits. The therapeutic activity is attributed to a complex mixture of phytochemicals, including terpenes, phenolics, carnosic acid, and carnosol. These constituents are known to inhibit the enzyme acetylcholinesterase (AChE) competitively, thereby increasing the availability of the neurotransmitter acetylcholine in the synaptic cleft, a mechanism similar to that of established Alzheimer's medications.
