SAGM blood refers to red blood cells (RBCs) that have been preserved in a Saline-Adenine-Glucose-Mannitol (SAGM) additive solution for transfusion. This solution is designed to extend the shelf life of red blood cells, typically up to 42 days, by providing essential nutrients and maintaining cellular integrity. Clinical studies have indicated that homologous SAGM blood transfusions may lead to immune compromise in patients, characterized by a reduction in NK cell activity and CD4:CD8 ratio, which can persist for several weeks post-surgery. This immune suppression has been associated with a higher incidence of post-operative nosocomial infections compared to non-transfused patients or those receiving autologous or leukocyte-depleted blood. Consequently, its use is advised to be judicious, with preference given to other blood product types when appropriate.
