Sahastara (SHT) is a Thai traditional polyherbal remedy included in the Thai National List of Essential Medicine, primarily used for the treatment of knee osteoarthritis. It is typically administered as an ethanolic extract or powdered capsule. Clinical evidence from Phase II trials indicates that Sahastara provides pain relief and functional improvement (measured by VAS and WOMAC scores) comparable to the NSAID diclofenac, but with a significantly lower risk of systemic side effects, particularly regarding liver enzyme elevations (AST, ALT, ALP) and blood pressure changes.
