Saireito is a traditional Japanese Kampo medicine composed of a standardized combination of two other formulas: Shosaikoto (Minor Bupleurum Decoction) and Goreisan (Five-Ingredient Powder with Poria). It is widely utilized in Japan for its immunomodulatory, anti-inflammatory, and diuretic properties, particularly in the management of inflammatory conditions associated with fluid imbalance. The formula consists of 12 medicinal herbs, including Bupleurum root, Alisma rhizome, and Scutellaria root. Saireito is frequently employed as a steroid-sparing agent in the treatment of renal diseases such as nephrotic syndrome and IgA nephropathy, as well as for gastroenteritis and edema. Its mechanism of action involves the suppression of pro-inflammatory cytokines like TNF-α and IL-6, and the regulation of water homeostasis through the modulation of aquaporin-2 expression in the kidneys.
