saline + N-acetylcysteine

Saline + N-acetylcysteine is a combination therapy consisting of intravenous hydration with isotonic saline and the antioxidant N-acetylcysteine. This regimen serves as the conventional standard-of-care control arm in clinical trials conducted by the Tel-Aviv Sourasky Medical Center, most notably the REDUCE-AKI trial (NCT01866800). The treatment is administered to high-risk patients, such as those undergoing Transcatheter Aortic Valve Implantation (TAVI), to prevent contrast-induced nephropathy (CIN) and acute kidney injury (AKI). Saline provides volume expansion to maintain renal perfusion, dilute contrast media within the renal tubules, and accelerate its excretion. N-acetylcysteine acts as a precursor to the endogenous antioxidant glutathione and directly scavenges reactive oxygen species, thereby protecting renal tubular cells from oxidative stress and direct toxicity induced by iodinated contrast agents.
