Samarium-153 oxabifore is a radiopharmaceutical developed for the palliation of bone pain in patients with metastatic bone disease, particularly those exhibiting widespread osteoblastic lesions. It is chemically similar to samarium-153 lexidronam (EDTMP), but uses a slightly different chelating ligand (ETMP) to avoid patent restrictions associated with EDTMP. The drug consists of radioactive samarium-153 complexed to a phosphonate ligand that targets areas of increased bone turnover—such as sites of metastases—where it emits beta radiation locally to relieve pain. Its clinical efficacy and safety profile are considered nearly identical to those of the original EDTMP-based agent[6][4][5]. Samarium-153 oxabifore is primarily used in regions where the original formulation is unavailable or cost-prohibitive due to intellectual property constraints[6].
