Samarium Sm-153 DOTMP (also known as TLX090 or CycloSam) is a clinical-stage, bone-seeking targeted radiopharmaceutical originally developed by IsoTherapeutics Group and currently owned and developed by Telix Pharmaceuticals (following its acquisition of QSAM Biosciences). The drug consists of the beta-emitting radioisotope samarium-153 complexed with the macrocyclic chelating agent DOTMP (1,4,7,10-tetraazacyclododecane-1,4,7,10-tetramethylenephosphonic acid). DOTMP has a high affinity for hydroxyapatite, a major component of the bone matrix, allowing the radiopharmaceutical to selectively localize to areas of high bone turnover, such as osteoblastic metastatic lesions and primary bone tumors. Once localized, samarium-153 delivers localized cytotoxic beta radiation to destroy cancer cells and alleviate bone pain while minimizing off-target toxicity to surrounding healthy tissues. Samarium Sm-153 DOTMP is currently in Phase 1 clinical development for the treatment of bone metastases (including pain palliation) and primary bone cancers such as osteosarcoma and Ewing sarcoma.
