Samarium sm-153 lexidronam + docetaxel is a combination therapy investigated primarily for the treatment of pain associated with bone metastases in cancer patients, particularly those with castration-resistant prostate cancer. Samarium sm‑153 lexidronam is a radiopharmaceutical that consists of radioactive samarium (samarium‑153) chelated to ethylenediaminetetramethylenephosphonic acid (EDTMP), which targets areas of increased bone turnover such as osteoblastic bone metastases. Upon intravenous administration, it accumulates preferentially in metastatic lesions and emits beta radiation, providing localized pain relief by damaging tumor cells and reducing nerve stimulation[1][3][5]. Docetaxel is a taxane-class chemotherapeutic agent that inhibits microtubule depolymerization, thereby disrupting cell division and inducing apoptosis in rapidly dividing cancer cells. Clinical studies have shown that this combination can be administered safely at full doses over repeated cycles, with observed responses even in taxane-resistant disease[2][4]. The primary indication for this regimen has been palliative care for metastatic bone pain; however, as of October 2021, commercial production of Quadramet (samarium sm‑153 lexidronam) was discontinued[6].
