Samuraciclib + gemcitabine + nab-paclitaxel is an investigational combination chemotherapy and targeted therapy regimen being evaluated for the treatment of metastatic pancreatic ductal adenocarcinoma (PDAC). The regimen consists of **samuraciclib** (CT7001), an orally bioavailable, selective small molecule inhibitor of Cyclin-Dependent Kinase 7 (CDK7); **gemcitabine**, a nucleoside analogue antimetabolite; and **nab-paclitaxel** (Abraxane), an albumin-bound nanoparticle formulation of the microtubule inhibitor paclitaxel. CDK7 is a critical regulator of both the cell cycle and RNA polymerase II-mediated transcription, and its inhibition is hypothesized to be particularly effective in the 'basal-like' molecular subtype of pancreatic cancer, which typically exhibits a more aggressive phenotype and poor prognosis. This triplet combination is currently being investigated in a Phase Ib/II clinical trial to determine the safety, tolerability, and preliminary efficacy of adding a CDK7 inhibitor to the standard-of-care gemcitabine/nab-paclitaxel backbone.
