San Huang Xiao Yan Fang (三黄消炎方), also known as the "San Huang Xiao Yan recipe" or "SHXY," is a traditional Chinese medicine (TCM) herbal compound used primarily for its anti-inflammatory properties. It has been widely applied in clinical practice for several decades, especially in the treatment of diabetic foot and diabetic ulcers. The formula is composed of multiple herbs (the exact composition may vary by source but typically includes Coptis chinensis [Huang Lian], Scutellaria baicalensis [Huang Qin], and Rheum palmatum [Da Huang]), which are known for their heat-clearing and detoxifying effects. Mechanistically, SHXY exerts its therapeutic effect by modulating inflammatory pathways. It activates the AMPK/Nrf2 pathway to suppress abnormal inflammation via inhibition of HMGB1 expression. In animal models and cell studies, it upregulates Nrf2 and AMPK while downregulating HMGB1, leading to reduced inflammation and accelerated wound healing[1]. Additionally, it inhibits Th17 cell differentiation through modulation of CD4+ T cells without affecting Treg or Th1 cells; this involves pathways such as TNF signaling, IL-17 signaling, HIF-1 signaling, PI3K-Akt pathway[3]. These actions contribute to improved outcomes in conditions characterized by chronic inflammation such as diabetic foot ulcers.
