Overview

Sangzhi alkaloids is a botanical drug derived from the twigs of *Morus alba* L. (Mulberry twigs), which was approved by the National Medical Products Administration (NMPA) of China in 2020 for the treatment of type 2 diabetes mellitus. It is a refined extract containing a group of polyhydroxy alkaloids, with 1-deoxynojirimycin (DNJ), fagomine, and 1,4-dideoxy-1,4-imino-D-arabinitol (DAB) as the primary active components. The drug acts as an alpha-glucosidase inhibitor, delaying the digestion and absorption of carbohydrates in the small intestine, thereby reducing postprandial blood glucose levels. Beyond its primary indication, it is being investigated for its potential to improve metabolic disorders in perimenopausal women, including glucose and lipid metabolism abnormalities, both as a monotherapy and in combination with menopausal hormone therapy.
