Overview
Sanmiaoshan (also known as Sanmiao San) is a classic Traditional Chinese Medicine (TCM) herbal formula composed of three primary ingredients: *Cortex Phellodendri* (Huang Bai), *Rhizoma Atractylodis* (Cang Zhu), and *Radix Achyranthis Bidentatae* (Niu Xi). It is traditionally used to clear heat and dry dampness, particularly in the lower body, and has been clinically investigated for its potential in treating Benign Prostatic Hyperplasia (BPH) and associated lower urinary tract symptoms (LUTS). The formula's therapeutic effects are largely attributed to isoquinoline alkaloids, most notably berberine found in *Cortex Phellodendri*, which exhibits anti-inflammatory properties and potential alpha-adrenergic blocking activity. In clinical research, it is often evaluated as an adjunct to standard therapies like alpha-blockers or in combination with other dietary supplements like saw palmetto.
