Sanwujiaowan is a Traditional Chinese Medicine (TCM) formulation traditionally used for the treatment of cold-dampness Bi-syndrome, which encompasses conditions such as rheumatoid arthritis, osteoarthritis, and chronic joint pain. The formula is composed of three primary herbal ingredients: *Aconitum carmichaelii* (Chuanwu), *Aconitum kusnezoffii* (Caowu), and *Zanthoxylum bungeanum* (Huajiao). Its therapeutic efficacy is derived from the synergistic action of its chemical constituents, most notably aconitine-type alkaloids and alkylamides such as sanshools. Aconitine acts as a potent modulator of voltage-gated sodium channels, prolonging their open state to exert analgesic effects, while sanshools interact with TRPV1 and KCNK potassium channels to produce a characteristic numbing sensation and further pain relief. Due to the inherent toxicity of *Aconitum* species, the herbs are typically processed (paozhi) to reduce alkaloid concentrations before formulation. It is manufactured by several traditional pharmaceutical companies in China and is recognized for its potent anti-inflammatory and antinociceptive properties.
