SAR113945 is a small molecule inhibitor of IκB kinase, developed for intra-articular injection in a slow-release formulation to treat symptomatic osteoarthritis of the knee. The drug specifically inhibits the IκB kinase complex, which plays a key role in inflammatory signaling pathways. In preclinical studies, SAR113945 demonstrated inhibition of interleukin 1β, tumor necrosis factor α (TNFα), and prostaglandin E2 synthesis. It showed positive effects on thermal and mechanical hyperalgesia in animal models and was superior to triamcinolone in some measures. Clinical trials included several phase 1 studies confirming safety and tolerability, but subsequent phase 2a results failed to show efficacy for the primary endpoint in the overall population with knee osteoarthritis. Development was discontinued by Sanofi[1][4][6].
