SAR407899A is an orally administered small molecule inhibitor of prolyl oligopeptidase (POP), also known as prolyl endopeptidase (PREP), developed by Sanofi for the treatment of chronic kidney disease (CKD). The drug's mechanism of action involves the inhibition of POP, the primary enzyme responsible for the degradation of N-acetyl-seryl-aspartyl-lysyl-proline (AcSDKP). AcSDKP is an endogenous tetrapeptide with significant anti-fibrotic and anti-inflammatory properties. By increasing the levels of AcSDKP, SAR407899A was intended to mitigate renal fibrosis and inflammation, potentially providing synergistic benefits when used alongside standard-of-care angiotensin-converting enzyme (ACE) inhibitors.
