SAR443809 is a humanized monoclonal IgG4 antibody that selectively inhibits the complement alternative pathway by binding to the activated form of complement factor B (factor Bb) and blocking the cleavage of C3 and factor B. This mechanism effectively inhibits the activation of the alternative pathway C3/C5 convertase (C3bBb), leaving the initiation of the classical and lectin complement pathways unaffected. Preclinical and early clinical studies suggest strong potential for treating alternative pathway-mediated disorders, particularly paroxysmal nocturnal hemoglobinuria (PNH)[1][2][3][5][7]. The drug is administered intravenously or subcutaneously and has demonstrated sustained pharmacodynamic effects, including inhibition of hemolysis and C3b deposition on erythrocytes in relevant disease models[3][5][7]. Developed by Sanofi, SAR443809 is currently in phase 1 clinical trials for PNH.
