SAR446309 is an investigational bispecific nanobody (VHH) developed by Sanofi that simultaneously targets Tumor Necrosis Factor (TNF) and OX40 Ligand (OX40L). This dual-targeting approach is designed to provide enhanced therapeutic efficacy in inflammatory diseases by combining the rapid anti-inflammatory effects of TNF inhibition with the T-cell modulating effects of OX40L blockade. TNF is a key driver of systemic inflammation, while OX40L is a costimulatory molecule that promotes T-cell activation and survival. By inhibiting both pathways, SAR446309 aims to achieve deeper clinical responses in conditions like hidradenitis suppurativa, where TNF inhibitors alone often provide incomplete relief. The molecule was developed using the Ablynx Nanobody platform, which utilizes single-domain antibody fragments derived from camelids.
