Sarabesin 3 (SB3) is a radiolabeled peptide-based imaging agent designed as a gastrin-releasing peptide receptor (GRPR) antagonist. Developed primarily by Erasmus MC (Universitair Medisch Centrum Rotterdam), it is typically labeled with the positron-emitting radionuclide gallium-68 (68Ga) for use in positron emission tomography (PET) imaging. The agent targets the GRPR, which is frequently overexpressed in various malignancies, including prostate and breast cancers. As an antagonist, Sarabesin 3 binds to the receptor without triggering a biological response, potentially offering a better safety profile and imaging contrast compared to earlier agonist-based tracers. It has been evaluated in clinical studies for the detection, staging, and intra-operative guidance of GRPR-expressing tumors.
