Sarcosine + selective serotonin reuptake inhibitor (SSRI) is a combination therapy involving the co-administration of sarcosine (N-methylglycine), an endogenous amino acid and glycine transporter-1 (GlyT-1) inhibitor, with an SSRI, a class of antidepressants that inhibit the neuronal reuptake of serotonin. Sarcosine acts by increasing synaptic glycine levels and enhancing N-methyl-D-aspartate receptor (NMDA receptor) function, thereby modulating glutamatergic neurotransmission. SSRIs increase serotonergic neurotransmission by blocking the serotonin transporter. This combination has been studied as an add-on treatment for major depressive disorder (MDD), schizophrenia, obsessive-compulsive disorder (OCD), and other neuropsychiatric conditions to improve efficacy over monotherapy. Clinical trials have shown that adding sarcosine to SSRIs can result in greater reductions in depressive symptoms and higher response/remission rates compared to SSRIs alone[1]. The combination may also be beneficial in treatment-resistant cases but requires careful dose adjustment due to potential synergistic effects on serotonergic transmission[5].
