Overview

Sarilumab + tocilizumab is a combination of two monoclonal antibody drugs, both of which are interleukin-6 (IL-6) receptor antagonists. Sarilumab (marketed as Kevzara) and tocilizumab (marketed as Actemra, among other names) are each approved for the treatment of moderate to severe rheumatoid arthritis and other inflammatory conditions. Both agents work by binding with high affinity to soluble and membrane-bound IL-6 receptors, thereby inhibiting IL-6–mediated signaling pathways that drive inflammation. This dual blockade may theoretically provide enhanced suppression of the IL-6 pathway in diseases characterized by excessive IL-6 activity, though clinical use of this specific combination is not standard practice and data on combined administration are limited. Each drug has been developed independently—sarilumab by Regeneron Pharmaceuticals/Sanofi and tocilizumab by Chugai/Roche—and both have demonstrated efficacy in reducing disease activity in rheumatoid arthritis through inhibition of pro-inflammatory cytokine signaling[1][2][3][5][7].
