Satavaptan is a selective, orally active vasopressin V2 receptor antagonist belonging to the vaptan class of drugs. Developed by Sanofi, it was primarily investigated for the treatment of hyponatremia, particularly in patients with the syndrome of inappropriate antidiuretic hormone secretion (SIADH), and for the management of cirrhotic ascites. By selectively blocking the V2 receptors in the renal collecting ducts, satavaptan prevents the action of arginine vasopressin (AVP), thereby promoting aquaresis—the excretion of solute-free water—without significantly affecting electrolyte balance. This mechanism helps to increase serum sodium levels and reduce fluid accumulation. Although the drug reached Phase 3 clinical trials, Sanofi discontinued its development in 2009 following a strategic portfolio review and assessment of the regulatory landscape.
