SB43-gal is a galactosylated monoclonal antibody designed as a clearing agent for use in Antibody-Directed Enzyme Prodrug Therapy (ADEPT). It specifically targets the active site of the bacterial enzyme carboxypeptidase G2 (CPG2). In the ADEPT clinical protocol, an antibody-enzyme conjugate (such as A5B7-CPG2) is administered first to localize the enzyme at the tumor site. SB43-gal is subsequently administered to bind and inactivate any residual enzyme remaining in the systemic circulation. The galactosylation of the SB43 antibody allows the resulting antibody-enzyme complex to be rapidly cleared from the bloodstream by the liver via asialoglycoprotein receptors. This selective clearance ensures that the later-administered prodrug is converted into its active cytotoxic form only within the tumor microenvironment, thereby reducing systemic toxicity and improving the therapeutic index.
