SBM-04 is a biosimilar Fc fusion protein currently in preclinical development by Shilpa Biologicals for the treatment of rheumatological conditions. It is a biosimilar candidate for **abatacept** (Orencia). The molecule is designed to function as a selective T-cell costimulation modulator, consisting of the extracellular domain of human cytotoxic T-lymphocyte-associated antigen 4 (CTLA-4) fused to a modified Fc portion of human immunoglobulin G1 (IgG1). By binding to CD80 and CD86 on antigen-presenting cells, it inhibits the costimulatory signal required for T-cell activation, thereby reducing the inflammatory response associated with autoimmune diseases such as rheumatoid arthritis.
