"SBRT + ipilimumab + sintilimab + AG chemotherapy" is an investigational multi-modal combination regimen being evaluated for the treatment of locally advanced pancreatic cancer (LAPC). The regimen integrates stereotactic body radiotherapy (SBRT), a precise local ablative technique, with dual immune checkpoint blockade and cytotoxic chemotherapy. Ipilimumab (Yervoy) is a monoclonal antibody targeting CTLA-4, while sintilimab (Tyvyt) is a monoclonal antibody targeting PD-1. The "AG chemotherapy" component consists of nab-paclitaxel (Abraxane) and gemcitabine (Gemzar), which is a standard-of-care doublet for pancreatic cancer. This strategy aims to leverage the immunogenic cell death and local control provided by SBRT alongside systemic therapy to overcome the highly immunosuppressive microenvironment of pancreatic ductal adenocarcinoma. It is currently being studied in a Phase II clinical trial sponsored by the Shandong Cancer Hospital and Institute.
